Medicaid is a government assistance program designed to help people cope and pay with their medical bills. It is jointly sponsored on federal, state, and city levels. The program is primarily designed for low income individuals are eligible for public assistance. In New York, however, some higher income individuals may meet eligibility requirements if they are disabled or as a supplement for seniors with high medical bills.

How to Apply for Medicaid in New York

In April of 2010, New York State introduced a new Medicaid application and it has been required in all districts as of June 11, 2010. The old applications are no longer accepted, so it is necessary to be sure you are given the new 6-page form, the revised DOH-4220. This new form was designed to replace the more complicated 16-page form required for long-term nursing care.

To receive the application or to receive assistance in filling it out, those in New York City can call the Human Resources Administration at 718-557-1399 or toll-free from any of the New York Boroughs at 877-472-8411. In addition, applications can be mailed to Initial Eligibility Unit, HRA/Medical Assistance Program, P.O. Box 2798, New York, NY 10117-2273.

Several documents and proofs are required with the application for Medicaid, including the following:

• Proof of age – birth certificate or passport.
• Proof of citizenship or resident status – birth certificate, passport, legal residence card.
• Proof of recent income from the past month.
• Bank account and insurance information for all those in the household over 65 or disabled.
• Proof of residence – lease, mortgage, letter from landlord, or official mail.
• Insurance policy or card, if applicable.
• Medicare card, if applicable.

Eligibility Determination for Medicaid in New York

For the majority of people, eligibility depends on the level of income in the household. For an individual, the maximum income for 2010 is $707 per month. For disabled individuals or seniors, the maximum income for 2010 is $767. Income levels are higher for families based on the number of people.

In some situations, it is possible to make more than the maximum income per month and still be eligible. This may be the case for the disabled, the blind, seniors over 65, children, and pregnant women. In some instances, it depends on how high the outstanding medical bills are for the individual or the family. For children and pregnant women the income adjustment can be as high as $1,805 per month.

Those who are eligible will be contacted by mail and they will receive a benefit card to use when services are delivered.

What is Covered by Medicaid in New York?

Most basic emergency and need-based medical services are covered by Medicaid. Coverage, however, depends on several factors, including income, age, and other household information. Some coverage may be available only with a personal co-payment. Coverage include the following:

• Treatment to quit smoking
• Preventative care
• Hospital services
• Lab work
• X-rays
• Nursing home stays
• Stays in mental health care facilities
• Child and teen health
• Medical equipment and appliances
• Transportation
• Emergency or ambulance transportation
• Pregnancy, pre-natal, and maternal care

Managed Care and Medicaid in New York

In some areas and in some situations, those eligible for Medicaid will be required to join a managed care program with a licensed Health Maintenance Organization. Some hospitals and clinics also offer managed care programs. This will entail choosing a primary doctor from which services must be used. The doctor will recommend a specialist if he or she cannot provide the level of care required. In some counties in New York, individuals are free to choose if they want to join a program, but managed care is mandatory for families on Medicaid.

We are 3 family members ( wife, husband and a child 14 years old).
My & my husband health insurance is active, My son (aleca norman) not more
active.
I renew all package and I do not know what happen.
Every child under 19-20 years old is
eligible for NYC medicaid.
Perhaps there is a problem in your system
please let me know
